Community Health Center Innovation Challenge

We are pleased to announce the grant awards for the Community Health Center Innovation Challenge. Congratulations to all the grantees! The purpose of this program to provide seed funding for innovative ideas, support out of the box thinking and test out new solutions to common problems in order to improve care. We are seeking to strengthen the capacity of clinics to undertake innovative practice transformations that are framed by the concepts of the Institute for Healthcare Improvement's Triple Aim of improved population health, better patient experience and reduced cost.

Health Home Innovation Fund

We are pleased to announce the grant awards for the Health Home Innovation Fund. Congratulations to all the grantees! The purpose of the Health Home Innovation Fund is to provide flexible funding to support local partnerships among private and public safety net providers and local health plans that are committed to pursuing more effective, integrated systems of care for traditionally underserved populations in California.
